County of Los Angles Department of Parks and Recreation - Facility Interest Form

Thank you for your interest in County Parks as a partner for your school usage. This new Memorandum of Agreement (MOA) is an update of our business practices and must be executed by every School District or School (public, private, or charter). The MOA contains an indemnification clause that will need to be accepted in addition to requiring Commercial Insurance for liability for the usage of County Parks facilities. Self-Insurance will be allowed for workers compensation and for property only. This process must be approved by your School Board, Board of Trustees, or proper governing body, as required by State law.
The information requested below is required to help complete a MOA. There may be fields that are not applicable, and they can be left blank. The most important information to help start the process is the contact information of a responsible Administrator that can negotiate and take the MOA to your School Board or Governing body.

This initial date/time request is preliminary only for Departmental planning purposes, and not a guarantee of availability. Once your MOA is final you will then have the opportunity to reserve your preferred date and time with our Central Reservations.
